Filing Dates & Requirements

Applications must be filled out completely. We will not accept incomplete applications. All questions must be answered in detail.

Fermented Malt Beverage and Intoxicating Liquor Licenses, Forms AT-115 and AT-106

Corporations must apply for licenses under the name for which they are registered with the Secretary of State's Office.
A Corporation must have at least two officers sign the application; namely a President and a Secretary who are not the same person.
The President and Vice-president of the corporation also cannot be the same person.
Every Corporation must have an appointed Agent.
Make sure that applications are complete.

Furnish a complete description of the licensed premise (rooms, buildings, and/or land area) where alcoholic beverages are to be sold, served, and stored. If your description is too long for the space provided on the form please use a separate piece of paper for the whole premise description and attach it to the renewal application. Premise descriptions will not be carried from the previous year. Licenses will be issued for the premise description provided on the application.

Please Note: Renewal form AT-115 or Original form AT-106 requires your Wisconsin Seller's Permit number and your Federal Employer Identification Number.
