Filing Dates & Requirements

All renewal alcohol licenses are due in to the clerk's office by April 15th. All beer & wine licenses are due in to the clerk's office by April 15th. 

First-Time Applicants Only (or Changes in Company) Requirements:

Original Alcohol Beverage License Applications

After you have completed the application, agent schedule and Auxiliary questionnaire for ALL officers AND agent, please submit all pages to the Clerk's Office.

Most Common: Renewal Alcohol Beverage License Application

After the application has been completed, submit both pages to the Clerk's office.

Wisconsin Department of Revenue now offers a fill and print Renewal Form on their website. Your complete premise description may be too large for the space they provide on their form. If this is the case please type the complete premise description on a single sheet of paper and attach to this form.

Filing Dates

All combination fermented malt beverage/intoxicating liquor licenses, Class C-Wine and Fermented Malt Beverage licenses are due in the Clerk's office on April 15th. 

Class C-Wine application must also include sales figures for the previous year.
