Filing Dates & Requirements

All renewal alcohol licenses are due in to the clerk's office by April 15th. All beer & wine licenses are due in to the clerk's office by April 15th.

Original and Renewal Alcohol Beverage License Applications (DO NOT use any previous applications you may have saved. You MUST use these current forms! Any old forms will NOT be accepted.)

After you have completed the application, appointment of agent and Individual Questionnaire for ALL officers AND agent, please submit all pages to the Clerk's Office. Please make a detailed and accurate description of the premise and where beer/alcohol will be stored and served.

Filing Dates

All combination fermented malt beverage/intoxicating liquor licenses, Class C-Wine and Fermented Malt Beverage licenses are due in the Clerk's office on April 15th. 

Class C-Wine application must also include sales figures for the previous year.
